This form is to be completed by any person whose personal information is contained within any documentation submitted with a particular planning permit application.

The Planning and Environment Act 1987 and the Privacy and Data Protection Act 2014 prohibits the disclosure of certain personal information in a document that is made available on an Internet site without the individual 's consent.

Note: this prohibition does not apply when a document is made available in another way (for example for physical inspection at an authority 's office or emailing an electronic copy to a person).
