Request a transfer form for a health business

When a health business changes ownership, you need to transfer the registration into the new proprietor's name.

Food businesses can no longer transfer registration - please refer to buying and selling a food business for the changes under the Food Act.

Health businesses include:

  • hair
  • beauty or
  • accommodation premises.

If a business is registered under the Public Health and Wellbeing Act you can still -

On receipt of your request we will email the form to you within working two days.

This only relates to Health businesses. Food Businesses can no be longer transferred under the Food Act. 

What happens next...

When you receive the form

  • Please read it carefully.
  • Complete all the information. Incorrectly completed forms can delay the process and potentially impact any settlement arrangements.
  • The form must be signed by both the incoming (new) proprietor and the outgoing (leaving) proprietor/s.
  • It should be returned with payment of the applicable fee - this is on the top of the form. It can be emailed to us.
  • The responsibility of payment of the fee is arranged between the two parties.
  • Pay the invoice.
