This form must be completed for any proposed works to enhance or upgrade a Council facility.

Proposed works include anything that might potentially be fixed to an asset or ground for example: sports lights facility or pavilion sporting infrastructure turf management etc.

The proposal for works must be approved before the commencement of any project.
