The sale of food that is considered to be low risk is referred to as Class 4.
This form should be used for Class 4 fixed food premises.
A fixed food premises is a permanent building that sells low risk foods. It does not include food sold from a stall or marquee or vehicle or cart.
You can use the Department of Health Business classification information if you do not know which Class your food premises would be considered.
