Support at Home Program

The Australian Government's Support at Home program is designed to help older people remain living independently and safely in their own homes for longer.

Support at Home provides access to a range of personalised services that are tailored to an individual's needs, goals, and circumstances. Services may include:

  • Personal care (showering, dressing, grooming)
  • Domestic assistance (cleaning, laundry, meal preparation)
  • Shopping and transport
  • Nursing and allied health services
  • Home maintenance and modifications
  • Social support and community connection
  • Assistive technology and equipment
  • Respite and support for carers

Finding a Support at Home Package Provider in our Community

There are many Home Care Package Providers in the Greater Geelong region. We encourage you to use the Find A Provider function on the My Aged Care website to find a suitable provider.

Living at home is much more than just keeping your house clean and tidy. Our service aims to enrich your life and draws upon an extensive list of services through our preferred provider network for example:

  • Home visiting services - Podiatry Occupational Therapy Physiotherapists
  • Support Services - showering and dressing home care social support respite shopping assistance
  • Orthotics
  • Basic Gardening services to keep the yard safe and maintained

  • Food preparation

  • Planned activity groups and socialisation programs - Men’s shed

Our Care Partners are experts in helping our SAH clients to understand how to use their SAH support appropriately to maximise independence and support to live at home.

 

Fees and Contributions

Support at Home services may involve a contribution from participants, depending on the type of service received and an individual's financial circumstances.

The Australian Government determines participant contribution arrangements, which are designed to ensure services remain affordable while supporting access to quality care. Some services may attract a higher contribution than others, and certain participants may qualify for reduced fees or financial hardship assistance.

Before services commence, we will provide a clear explanation of any fees that may apply and ensure you understand your options. We are committed to transparent pricing, with no hidden costs, and will work with you to make the most of your available funding.

Eligibility

The Support at Home program is available to older Australians who have been assessed as needing support to continue living safely and independently at home.

To be eligible, you generally need to:

  • Be an older person with care and support needs.
  • Be aged 65 years or older (or 50 years or older for Aboriginal and Torres Strait Islander people).
  • Complete an assessment through My Aged Care 1800 200 422
  • Be approved for government-funded aged care services.

Following an assessment, eligible participants are assigned a funding classification based on their assessed care needs. This funding can then be used to access a range of in-home services that support independence, health, wellbeing, and quality of life.
